| 1. | Go to Settings > Form Setup > Form Builder. |
| 2. | Select the Form you want to edit |
| 3. | Select the Step "Edit Pages" |
| 4. | Select the Page that you want to add the question to. |
| 5. | Click the Module button with in the "Click to Add" buttons row: |
| 6. | Select module type: |
Login: Requires the customer to login to the form.
Online Account Sign up: Give the customer the ability to create an online account.
Waiver: Create a waiver for your customers to read.
(Other Modules are available depending on the page)
| 7. | A pop up box will appear which will allow you to set the settings for the module. Once you are finished click the gold save button. |
